Here is an opportunity to be part of the Cantorial selection process. Your opinion is very important to the committee. Please download, print and fill out the complete form the best you can. If you cannot remember the Cantor's name, the date that he/she attended will suffice. You are to rate each section from 1-5 with 5 being the highest rating.When completed, mail your form to GHC or bring it to GHC and place it in the GHC office box to the attention of Phil Sassoon.
